Ever want access to your immunization record?
It’s now easier than ever.
Immunization Connect Ontario (ICON) is a secure online tool where you can view or print your immunization record.
You also can submit new immunizations to keep your record up to date.
With ICON, you can access your immunization record any time you’d like, from anywhere.
An immunization record is a document that provides the dates and names of immunizations you have received.
This also may be referred to as your “yellow card.”
Those who should sign up include:
•anyone who wants quick access to their immunization record;
•people travelling to another province or country;
•children registering for day care;
•students registering for school;
•people applying for a new job; and
•anyone who visits a family doctor.
To sign up, call your local Northwestern Health Unit office to get your Ontario Immunization ID and to set a PIN number.
